San Gabriel Hospice has a responsive admissions team. Serving Los Angeles County, San Bernardino County, Riverside County and Orange County, we are a preferred provider for end-of life care and education in the greater Los Angeles area.

For Skilled Nursing facilities

San Gabriel Hospice plays a supportive role to your staff, residents and their families in end-of-life care and comfort measures. We coordinate with your current caregivers and are happy to provide educational assistance.
